SCHEDULE FORM A (Rule 4(2), Solicitors' Remuneration (Enforcement) Rules 2004)
NOTICE TO COMPLETE AND DELIVER A RETURN
To:
Name of advocate and solicitor (or firm of advocates and solicitors)

For the purposes of obtaining full information for ascertaining whether or not an advocate and solicitor has complied with the Solicitors' Remuneration Order (and any rules or rulings of the Bar Council pertaining to the Solicitors' Remuneration Order), the Bar Council requires you to complete and deliver to the Bar Council within [state time period of not less than 30 days] from the date of service of this notice a return in Form B annexed hereto ("the return") relating to the transactions for the period commencing [state date] and ending [state date] (both dates inclusive).

All terms and expressions used in this notice and in the return shall have the meanings ascribed to them in the Solicitors' Remuneration (Enforcement) Rules 2004.

For the purposes of the return, “transactions” mean any payment of fees to you in relation to your completed non-contentious business for the period(s) specified in paragraph (1) above [or (delete as appropriate) the non-contentious business undertaken or performed by you [describe the nature or manner of such business] for the period(s) specified in paragraph (1) above];

The return shall be accompanied with a copy of the documents specified therein. The return together with the requisite documents shall be sent to the Secretary of the Malaysian Bar at the Bar Council Malaysia, No. 13, 15 & 17 Leboh Pasar Besar, 50050 Kuala Lumpur.

Upon receipt of the return, the Bar Council may require any advocate and solicitor to attend personally before the Bar Council and produce for examination all documents relating to the transactions or to provide in writing such information or particulars that the Bar Council deems necessary.
6.
Failure to comply with this notice or any other requirement under the Solicitors' Remuneration (Enforcement) Rules 2004 will render an advocate and solicitor liable to disciplinary proceedings under Part VII of the Legal Profession Act.
